This is exceptionally disappointing. The color of the design is not at all similar to the proof. The yellow color looks burnt orange and the red color looks nearly black like the fabric. Due to the way it was shipped, the graphic was folded on itself. Because of this, the ink stuck together, and there are now multiple small holes in the print as a result of separating the areas stuck together. This issue is trivial to prevent by simply adding a thin sheet of parchment over the print so it doesn't stick to itself when folded. If customers are expected to approve a proof, then the customer should expect the item to look as such. If a customer 'approved' a poor quality proof you would hold them accountable to that as they 'approved' it. Where is your accountability to deliver a product the customer expects based on the proof you provided?
