The designer didn’t follow my instructions, and bad customer service

I specifically requested my design not have a background, when I received my proof, it appeared that there was a clear background, so I approved my order. When it arrived I found out the background was actually white, and would not work for the application I needed. The customer service agent told me they saw my request and wasn’t sure why it was not followed, yet still did not offer me a refund.
